CP deaths

I know this has been asked before, but can someone please tell me how the one person died when it was CP's fault. My friends and I could not remember.

Don't seem to recall about any deaths attributed to CP's fault. Someone had a heart problem and died after getting off of a ride. Maybe someone else could shed some light here.
